provide infomation regardng the name, address and driving license number of the person who was driving thevehicle, when the below
mentioned offence was alleged to have been committed within 7 days from the receipt of this notice.Failure to furnish this information is
an offence under section 177 of the Motor Vehicle Act which is punuishable with fineof amount given below or three months
imprisonment, in addition to punishment provided for violation of traffic law/rules.In case, you wish to compound the traffic offence, you
may please deposit the below mentioned compounding fine on behalfthe erring driver at the Traffic Compounding Booth at the office of
the Supdt. of Police - Traffic, Yatayat Bhawan, Gorakhpur under proper acknowledgement within 7 days. For any Clarification, the
Owner/Driver should contact the notice Supdt. ofPolice - Traffic, Yatayat Bhawan, Gorakhpur with relevant document in original. In case of
