Dear Sir:

I hope this letter finds you in good health and high spirits. I am writing to inform
you that I have undergone a heart angiography procedure on [01-June-2024]. Due to the
seriousness of my condition and the nature of the procedure, my doctor has advised me to
take complete bed rest and strictly avoid any physical activity, including walking.

Attached to this application is the medical certificate from my attending physician, which
outlines the necessity of my bed rest and the expected duration of my recovery period.
According to the doctor's advice, I am required to remain on complete bed rest for a period.

Given the circumstances, I kindly request you to grant me medical leave. I assure you that I
will keep you updated on my health progress and will provide any additional medical
documentation if required.

I understand the inconvenience my absence may cause and I appreciate your understanding
and support during this time. I will ensure that my responsibilities are managed in my
absence and will make every effort to resume my duties as soon as I am medically cleared to
do so.
