Dear Sir,

We have applied for registration of our new firm WELL DRESS GARMENTS, Block 02,
Purani Ghala Mundi, Dera Ghazi Khan. in your office and deposited registration fee at RS.
_______ in the National Bank of Pakistan, Dera Ghazi Khan, as per partnership Act, 1932,
under the head of account of the provincial government. If the said head of account is
changed or otherwise said deposit is declared illegal, we will be bound to deposit the said
registration fee under the district government’s head of account or in any other head of
account as is determined by the government or concerned authority. It is, therefore,
requested for the registration of our firm on the aforesaid terms/undertaking.
