III. Problem Presented
The client used alcohol, methamphetamine (shabu), and marijuana. But his drug
choice is marijuana. He was positive in the random drug test on May 2024 and felt down
because he was fired from work as a manager at MCDO Inc. due to this reason. He was
listed in PDEA for drug use and on August 4, 2024 he was referred to recovery center
at Serene Valley Recover Center. The client is feeling upset to his parents because he
was brought to the recovery center without knowing their reasons.
IV. Case Background
The client has a history of using alcohol, marijuana, and methamphetamine
(shabu), with marijuana being his substance of choice. However, he claims that he does
not feel overly dependent on substance use. He typically used these substances at home,
with neighbors, at school, and in forests.
He began using marijuana at the age of 18 during his college years. In his college
years, he sometimes attended classes while using marijuana. He described his college
campus as resembling a "mini forest," with hidden spots where he and his friends could
use marijuana without being caught. Despite this, he claims that marijuana did not
negatively impact his academic performance and even made him more participative in
class. He also mentioned that more than 100 students in his batch were involved in
substance use. His last reported use of marijuana was in May 2024.
The client also drinks alcohol, typically with friends. During college, he
occasionally skipped classes due to drinking. He mentioned having a girlfriend with
whom he would sometimes drink, along with other friends. He observed that many
people his age in Kidapawan City engaged in drinking alcohol and using marijuana. His
last consumption of alcohol was in July 2024 at his uncle's house in Bukidnon.
And later, at the age of 20, the client started using methamphetamine (shabu).
Before when he was still working at Shoppe Express, he used shabu to help him sleep,
as his shifts ran from 10 p.m. to 10 a.m., and he struggled to rest afterward.
During the presidency of Rodrigo Duterte, the client experienced two years of
abstinence. The war on drugs is the intensified anti-drug campaign that began during
this which reduced drug proliferation in the country, but later engaged in drug use after
the term of PRRD. He resumed using marijuana, largely influenced by friends during
outdoor activities.
He worked as a manager at McDonald's for nine months, earning Php 17,000 per
month, but lost his job after testing positive during a random drug test. In March 2023,
he underwent an administrative hearing and later was placed on preventive suspension
in April. In early May, he sent a termination contract, but he was not initially fired due
to his close relationship with the general manager. However, when a new general
manager took over, his case was revisited, resulting in significant distress, leading him
to fell down and grieve for three weeks.
The client expressed frustration about being referred to a recovery center by his
parents, as he believes they sent him due to his weight loss and suspicion of drug use.
And that is why is he was brought in at Serene Valley Recovery Center on August 4,
2023. Prior to being referred to the center, he had been waiting for a job opportunity
abroad, arranged by a friend in Singapore whom he met online. He expects this job to
be hiring in October and has requested to his parents to stay at the recovery center for
only two months. He admitted that he would be very angry with his parents if they did
not keep their promise regarding his stay at the center.
a. Client’s background
He is tall standing at 5'9" with brown skin and round eyes, Kian has a strong love
for outdoor activities, particularly mountain climbing with his friends. He is also
passionate about cooking. In 2013, Kian began his studies in electrical engineering but
pursued it for only one year before transferring to Cagayan de Oro, where he studied
financial management for two years. He then returned to Kidapawan City and enrolled
at the University of Southern Mindanao, where he earned a degree in Food Technology
(Food and Beverage Preparation Management) over four years.
b. Family background
Kian’s family currently lives in a subdivision in Kidapawan City. His parents are
legally married and maintain a strong relationship. His father works as a police officer,
which is the family's primary source of income. Kian has three siblings; he is the eldest.
Two of his siblings are already working, while the youngest is in her second year of
college. Kian grew up in a family that wasn’t very expressive, often experiencing silent
treatment from his family.
c. Community
He currently resides in an old subdivision, a sub-standard housing within a
diverse community. The neighborhood includes people from various ethnic groups, such
as Obo Manuvu, Maranaw, Bisaya, Cebuano, Ilonggo, and Ilocano. The Obo Manuvu,
Cebuanos, and Hiligaynons are the predominant groups in Kidapawan City. Cebuano
and Hiligaynon are the primary languages spoken in the community, which mostly
consists of residents who originally hail from Bohol. Christianity is the dominant
religion, although there is a significant Muslim population in the area as well. The
community has access to abundant water, basic healthcare facilities (though these are
located far from the subdivision), and an active 911 emergency service. The nearest
school is 1.5 kilometers away from the subdivision. According to Kian, many people his
age who live in the subdivision are involved with drugs. Social activities among the youth
often revolve around outdoor "gala" gatherings, frequently accompanied by alcohol and
substance abuse. His neighbors also engage in drug use, with some openly using
marijuana. These environmental factors played a role in influencing Kian’s own
involvement with drugs.
V. Assessment
The client is accustomed to a dynamic of silent treatment. He grew up not being
open with his family about his personal life and feels uncomfortable expressing his
emotions, often finding it awkward or “cringe”. Based on observation, the client is silent
during lectures but listens attentively, and also participative and good at painting and
drawing during art therapy. However, he is determined to and has committed to quitting
drugs after realizing that his substance use cost him his job, he has come to understand
the harmful effects of drug use and the negative path it would lead him if he continued.
He is now adjusting to the structured environment and working towards abstinence.
The client is also motivated to live independently, secure stable employment, and build
a better life for himself. He also recognizes the need to step outside his comfort zone in
order to create a future aligned with his own vision.
The client exhibits emotional distance and uses marijuana to cope with emotional
discomfort, while also denying the nature of his addiction. Most importantly, the client
feel resentment towards his parent. According to Karen Horney’s Psychoanalytic Social
Theory, childhood experiences can significantly impact adulthood. A client who feels
anger and distance from their parents may be grappling with unresolved emotional
conflicts stemming from unmet expectations, needs, and perceived neglect. In this
context, the client demonstrates basic hostility toward his parents, rooted in frustration
over their decision to enroll him in rehabilitation. This conflict has led to basic anxiety
and unhealthy coping mechanisms regarding his emotions and personal issues.
Furthermore, the client's responses indicate the use of defense mechanisms proposed
by Anna Freud. First, he exhibits denial by refusing to acknowledge feelings of
resentment toward his parents and his condition regarding marijuana use. Second, he
displays displacement, directing his resentment toward others as a way to shift blame
for his situation. Lastly, he engages in rationalization, attempting to justify the absence
of negative effects from marijuana use. These theories highlight an underlying struggle
with neurotic needs for affection, control, or independence, suggesting that both
addiction and emotional distance are responses to unmet emotional needs. Although
maladaptive, these patterns represent attempts to manage profound emotional
discomfort.
