ACTIVITIES TO CONDUCT AND PROPOSE FOR ACHIEVING THE EXCELLENT PERFORMANCE INDICATORS:
1. FULLY IMMUNIZED CHILDREN-Tracking of defaulters; Intensify REC (Reach Every Choldren);Health Education and Community Awareness of the importance of vaccination
especially for the children under 5; and Conference with the BHW about Immunization and Vaccination
2. ANTE-NATAL CARE-Tracking of Defaulters; Conducting of USAPAN Sessions to the pregnant clients; Adequate supply of pregnancy kits; and Coordination with the
Baranggay Officials especially one who is currently assign in Health Program Committee (attending barangay sessions and involving to any certain activities)
3. POST-PARTUM VISITS-Health Education and community awareness about the importance of Post-partum visits more especially educating the post-partum mothers about the
post-partum complications; USAPAN Session; continue the utilization of call slip; and tracking of defaulters
4. EXCLUSIVE BREAST FEEDING-Continue the advocation of BF1KD, in coordination with the BNS; Health Education and community awareness about the importance of
Exclusive Breastfeeding for children below 6 months; Mandate of E.O. 51 in Barangay Resolution;Posting of Tarpaulins and Giving of brochures about BREASTFEEDING

Addendum:
- Through applying the SMART (Specific, Manageable, Attainable/Aimable, Realistic and Time-bounded) Strategy, it is estimated that the percentage of all certain
relevant health programs are in small number of percentage due to the fact as being compensated with Barangay Hingatmonan’s less population (as of 2020), as compared to
the other barangays of Balamban.
-As one of the NDPs being presently assign in the Municipality of Balamban, I handle three (3) barangays namely: Hingatmonan; Bayong; and Matun-og. However, I
prioritized the baranggay of Hingatmonandue to the proximity of the location to the Main RHU of Balamban ( formally as Balamban Health, Diagnostic and Birthing Center) and
aside from that purpose, it is estimated as the highest population among the two remaining assigned baranggays. Both Bayong and Matun-og are located in the distant
mountainous area, as the distance being estimated around 20 to 30 kilometers (distance travel: 1 hour and 15 minutes). The approximate population of both mountainous
baranggays, according to statistical source is about 980 and 1087, respectively.
