Dear Ma’am
This is in connection with your Building Permit Application dated 1-07-2019 under Application No.
2020-0007 for the Proposed Renovation and Extension of Residential Building at 5 Mother Teresa St.
Dela Costa Homes Barangka Marikina City.
Kindly be informed that after verification and evaluation of the construction plan/s and other submitted
documents, the following were noted:
    1. Complete box 4 of Building Permit form.
    2. Submit an earthquake hazard assessment report from PHILVOLCS due to the nearby
        west valley fault.
    3. Submit a copy of the 2019 real property tax receipts.
    4. Provide 1 car garage with a minimum size of 2.5m x 5m per City Ord. No. 073 Series of 2019.
    5. Reflect adjacent properties on the site development plan.
    6. Lot size on lot plan does not conform with the lot size of the site development plan
        and floor plans.
    7. Window openings of bedrooms must open directly to an open court, yard or street.
    8. Extend firewall at the rear side up to 0.4m above the roof line.
    9. Structural computations must be dry sealed by the Civil Engineer.
    10. Provide a frame model analysis.
    11. Provide a seismic analysis and reflect on the load combination of beams, columns
        and footing design.
    12. Provide a design analysis summary output conforming with the structural plans.
    13. Septic vaults must be water-tight sealed.
    14. Sanitary piping system does not conform with provisions of City Ord. No. 167 Series of 1998.
    15. See attached electrical evaluation slip.
In view of the foregoing, we regret to inform you that we cannot issue the subject permit until such time
you have corrected the noted deficiencies
We are hereby returning to you all the plans and other supporting documents with the advice that you
confer with your architect/civil engineer for the necessary corrections in compliance with the provisions
of the National Building Code of the Philippines (PD 1096). Please note that permanent markers must
be used for all corrections & must be countersigned by the corresponding architect /engineer.
We are hoping that this will be given preferential attention in order to facilitate the processing of your
permit/s at the soonest possible time.
