I am the above quoted person who is your employee but on secondment to National Government Constituency Development
Fund (NG-CDF) as from November 2013.

I have been adjudged as a person living with disability with a disability certificate NO Ref: NCPWD-44S5YKKJ issued on
13/1/25 by the National Council for Persons With Disabilities who is to be considered for retirement at the age of sixty-five (65)
years instead of sixty (60) years as prescribed by the persons with Disability Act No 14 of 2003.I also have a five (5) year Tax
Exemption certificate NO KRAPWD0237220225 for persons with disabilities dated 25/02/25 issued by Kenya Revenue
Authority. (Copies are attached)

In the backchop of the above, I write to you to give my current employer consent to allow me retire at the age of sixty-five (65)
years as a person living with disability as per section 70(2) (b) of the Public Service Commission Act.

Your expeditious action will be highly appreciated.
