hereby grant permission for my child to join the firing activity of the 528th DMST Air Force
ROTC on either May 24,2025 (Saturday) or May 25, 2025 (Sunday), at the Negros Oriental
Police Provincial Office in Agan-an, Sibulan, Negros Oriental. I understand that there will be
medical personnel present to address any unforeseen accidents, and I am assured of my child’s
safety.
