I, JULIE ANN T. FLORES of legal age, Filipino, single, and a resident of 7874 Mckinley
Street Barangay Pio del Pilar, Makati City, after having sworn in accordance with law, hereby
deposes and states the following:

1. I am the same person executing this affidavit;

2. I am an applicant for membership for AKAP, Makati CI

3. My monthly income is fourteen thousand six hundred seventy-eight (14,678) pesos;

4. The said wage is so meager and only enough to cover my family’s needs;

5. I am executing this affidavit to attest to the truth of foregoing facts and for whatever legal
purpose this may serve best.
