PREPARING FOR ICD­10


On October 1, 2015, the healthcare industry will be moving over to ICD­10 (International Classification of Diseases 10 th
Revision) system. Complete and accurate codes will be necessary to submit claims. It is imperative that the appropriate ICD­
10 diagnosis code is noted on all requisitions. Failure to utilize the new coding system will result in unnecessary calls to your
office as Physicians Laboratory will be unable to process the insurance claim without this information.

Resources for ICD­10 Transition:


Medicare has developed a site to assist Physician practices with this transition. The web address is http://www.roadto10.org.
This website will help your office build a customized action plan for implementation. Additionally, it provides specific
references based on your type of specialty (Family Practice, Pediatrics, OB/GYN, etc). There are videos, webcasts and
template library’s available as well.

PAIN MANAGEMENT TESTING


Several physicians have expressed interest in an affordable comprehensive pain management screen. ARUP now offers two
panels that are performed using Time­of­Flight Mass Spectrometry and Enzyme Immunoassays that encompass all pertinent
drug classes needed to monitor patients that are in pain management programs.

DISCONTINUED TESTS ~ EFFECTIVE IMMEDIATELY

#405 Leukocyte Alkaline Phosphatase (LAP)


The LAP test historically has been used to screen for chronic myelogenous leukemia (CML) and polycythemia vera
(PV). The assay has poor sensitivity with other nonmalignant conditions causing false positive results. Newer, more
specific assays have been developed and have replaced this obsolete test.
When working up a possible CML diagnosis, the recommendations are to test for:
� Qualitative RT­PCR for BCR/ABL
� FISH for BCR/ABL
� Karyotyping
To help establish the diagnosis of PV, the recommended tests are:
� Erythropoietin
� JAK2 mutation analysis

Please submit only the blue swab for testing. The white swab should be used only for cleaning mucous from the cervix before
sampling with the blue swab. For males, this swab is not used and can be discarded. If the white swab is submitted, the tests
will not be performed and recollection will be necessary.

Nebraska Medicaid will not cover toxicology or semen analysis testing. If you want to order this testing on a Medicaid
patient, you must inform them that the test will not be covered under their insurance and have them complete an ABN
(Advanced Beneficiary Notice) prior to collection of the sample.
