Ref No : SZ/Electrical/BLR(R2)/70/VL/4055 Bengaluru -560071 Dated: 20.12.2024

Respected Sir,

We are submitting compliance report as follows.

1. Regulation 45(7):

Observation: The 500KVA, 11KV/433V, 3Phase, 50Hz, Dyn11 transformer installed near Exit gate no. 2 (Siddhi
Vinayak Transformer Industries)

Compliance Action Taken: Now it is completely stopped. We will submit application for your approval shortly.

2. Regulation 107(6) read with 103(1):

Observations:
i. The 02 nos. of 433V, 60HP Star/Delta starters with plug and socket arrangements connected to 02
nos. of 60HP Wire-saw machines installed in Quarry.

Compliance Action Taken: The required Earth Leakage Protection Devices (ELPDs) have been installed for the
above mentioned starters. Testing has been completed and the leakage settings have been set within the
stipulated range (200mA–400mA).

ii. 433V, 400A, SK high voltage equipment power distribution panel with 04 outgoing feeders

Compliance Action Taken ii: The 433V, 400A SK high voltage power distribution panel comprising 04 outgoing
feeders has been inspected and confirmed to be in safe operational condition. Each feeder has been properly
labeled and is provided with appropriate mechanical protection, earthlings, and isolation provisions. The Routine
checks are being conducted to ensure the safety standards maintained always.

iii. 03 no's of 433V feeders, controlled by Switch Fuse Units, feeding supply to apparatus installed in the
workshop and office building
Compliance Action Taken iii: All three 433V feeders connected through Switch Fuse Units have now been
thoroughly tested and upgraded to comply with safety norms. The SFUs have been fitted with suitable protective
devices to ensure secure supply and automatic fault handling for the electrical apparatus in the workshop and
office areas.

An Earth Leakage relay with provision to setting between 200mA to 400mA shall be installed and maintained in
order to automatically disconnect the supply in the event of Earth fault/ leakage and for automatic disconnection

Compliance Action Taken: An Earth Leakage Relay with adjustable settings ranging from 200mA to 400mA has
been installed and is being periodically tested for reliability. It is configured to automatically disconnect the
power supply in the event of earth leakage or Earth fault, thereby safeguarding personnel and equipment from
electric hazards.

3. Regulation 71(ii) read with 72(1)(i):

Observation: Guarding arrangement was found not provided under the 11KV Overhead Line crossing the road at
Exit Gate No. 02 of the mine.

Compliance Action Taken: The Guarding arrangement has been provided under the said overhead line with
proper earth connection at each discontinuity point, as per regulation.

4. Regulation 107(4) read with 107(6):

Observation: Switch Fuse Units did not have automatic disconnection provision.

Compliance Action Taken: New Earth switchgear with protective mechanisms has been installed to ensure
automatic disconnection in case of faults, ensuring safety of personnel and equipment.

5. Regulation 107(6):

Observation: Absence of single phasing preventer in 433V, 100A, 60HP Star/Delta starter panels.

Compliance Action Taken: Single phasing preventers have been installed on both starter panels to protect
against phase failure and ensure uninterrupted safe operation.

6. Regulation 102(1) & (3) read with 23(3):

Observation: Earth continuity was not maintained for 60HP wire saw machines.

Compliance Action Taken: The starters are now connected with properly earthed 3.5-core, flexible cables with
mechanical protection & Earth core is clearly marked and identifiable.

7. Regulation 117(2) read with DGMS Circular No. 05 of 2024:

Observation: Electrical supervisor with valid competency was not appointed.

Compliance Action Taken: Mr. Shivakumar. S. Hiremath is qualified Electrical Supervisor with a valid
competency certificate has been appointed as a part time supervisor and he is available as and when required.

8. Regulation 99(4):

Observation: Comprehensive Single Line Diagram was not updated.

Compliance Action Taken: The Single Line Diagram has been updated to reflect all current electrical installations
