AFFIDAVIT OF LOSS

I, Eliah Job F. Jardin, of legal age and residing/with address at 12 Peterjon Apr., Brgy. Dos,
Katipunan, Zamboanga Del Norte, under oath, state:

1. THAT on February 27, 2023 at Katipunan National High School, I noticed that my I.D.
has lost when I needed to present it to the entrance.

2. THAT I exerted diligent efforts to locate my idntification card or but to no avail and I
therefore considered the same as lost and can no longer be found;

3. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve and to request for replacement of same I.D.
from Katipunan National High School.
