Scope Based Practices Audit tool
1 Patients referred from other facilities should be seen by at least 1
step higher professional from referring clinician
2 Triage has to be done by at least GPs and above (Check letter of
assignment for the GP)

System redesign and EHSTG Boosters Audit tool
4 Triage system which is well equipped and facilitate one stop
triage, registration and cashier service and accommodate for
the needs of highly infectious cases
5 Diagnostic triage tool kit - BP apparatus, stethoscope, pulse
oximeter, glucometer, weighing scale
6 Referral clinics should be covered by the responsible specialist
or subspecialist
7 Evidence based Clinical Practice Protocols are prepared for
OPD Clinics
8 Pre-triage set up screen for highly infectious cases and isolate
them
9 pre-triage screening protocol
10 Cough corner and cough clinic
11 Protocol for patient channeling to the cough clinic
12 OPDs with good premises & lighting ( adequate windows and
doors to be opened)
13 Airborne and droplet precaution protocols
14 OPDs with needy furniture having at least one table and two
chairs
15 Medical equipment’s available at the OPDs level:- BP
apparatus, stethoscope, reflex hammer (weighing scare and
glucometer at least for department pools)
16 Functionality of hand washing basins with water & soap
Observe the availability of 70% of ABHR solution at each of
the OPDs ( CASH audit reports on hand hygiene section)
17 Outpatient department is managed by at least a GP and specialty
clinics by a service specific specialist/ sub- specialty clinic by
sub specialist
18 Adequate OPD waiting area for patients , proportional to the
number of OPDs and number of patients seen per day
19 OPD Services Protocol (Protocolize - start time, service and
academic activities to lunch time service)
 lunch time service :- client interview and Review for
staff assignment schedule
20 Block based appointment system is in place
 appointment schedule
 Conduct client interview
 Review for the appointment register
21 Divide Clinic work hours based, For specialty Service to
morning and afternoon hours
