STATEMENT OF COMMITMENT TO COMPLY WITH OSH REQUIREMENTS

The Management and the employee of DMAN CONSTRUCTION AND SUPPLY are committed to provide
the quality to satisfy clients/ partner’s requirements, in order to achieve this policy, the company shall:

 Have an effective and efficient engineering and construction management team in place;
 Comply and implement with high standards all occupational safety and health (OSH) laws and
regulation in all aspects and activities of its projects;
 Safeguard and improve public health of its workers, as an important business element;
 Apply latest information technology in implementing its project and related safety and health
programs; and
 Provide trainings and seminars to develop the capacity of its human resources;

Every staff shall be responsible to adhere to the company’s safety and health policy and implement the
provisions of its Safety and Health Program in accordance with the duties and responsibilities assigned to
each one; and in conformance to the policies and programs of the Project Owner. It is everyone’s
responsibility to contribute to the continuing efforts to promote safety and health and prevent the occurrence
the accidents.

CERTIFICATE OF APPOINTMENT

The management hereby designate ROMALITO N. CORTES as FIRST AIDER and SAFETY
OFFICER, respectively, and the latter had undergone CONSTUCTION SAFETY TRAINING conducted
by the Department of Labor and Employment (DOLE), Occupational Safety and Health Center (OSHC).

I – USAGE OF SAFETY PARAPHERNALIAS MUST BE OBSERVED PROPERLY:

a. Workers assigned in the Road Construction must wear their Reflectorized Shirts at all time
b. Workers assigned in the actual pouring of concrete must wear their construction boots at all time.
c. Workers assigned in the construction site must wear the proper protection, hard hat and safety shoes at all time.
d. Workers assigned in the exterior works or above 3.00 m high must wear proper protection, hard hat, safety shoes and
safety belt.
e. Children are not allowed to enter or play, near the construction area.

II – CONSTRUCTION SAFETY:

a. All Dump trucks drivers must fasten their seatbelts while driving their respective units.
b. Operators / Drivers must take signal from any one person in an emergency however a stop signed can be given by
anyone.
c. Wear gloves when handling/placing of boulders and during caging/fabrication of gabions, objects or substance, which
could cut, tear or burn hands.
d. Be sure that the hauled boulders are away from the working area to avoid sliding/rolling that may hit the workers.
e. Workers must use safety shoes during manual hauling of boulders.
f. Established approximate limit for machine operation and complete the excavation by digging.
g. Before any attempt made to excavate be sure that the equipment is place in the hard soil.
h. Provide extra oxygen for excavation over 1.5m, to ensure safety when sufficient occurred.

III – EQUIPMENT SAFETY:

a. All equipment’s must be inspected and required quality check before it is used.
b. Routine maintenance fueling and repair must not be performed while the equipment’s are in the use or power on.
c. All equipment’s must park properly to the designated parking area or parking lot.

IV – HOUSEKEEPING:

a. Waste materials shall be removed from the site/areas to avoid accident


b. Cleaning of Obstructions at the passageways.

V – SAFETY MEETING:

a. Safety meeting must be conducted before and after the work.


b. Strict compliance for the workers to use their PPE.
c. Remind the workers to be aware of the hazards.
d. Proper orientation for the use of PPE.

VI – FIRST AID AND MEDICAL SERVICES

First Aid Facilities:

a. Project shall be required to provide a first aid kit and should have been with minimum medical requirements.
b. Every project should have a clinic or first aid station.
c. Project should be provided with emergency vehicles to handle the transportation of ill and injured workers to the hospital.
d. For every project the company must have a certified first aider.
e. Every construction project the company must establish at least five (5) members of safety and health committee.

1. Company Safety and Health Policy

It is the general policy of DMAN CONSTRUCTION AND SUPPLY to perform work in the
safe manner possible consistent with good construction practices. To fill the requirements of this
policy, an organized and effective Construction Safety Health Program as prescribe in this
program and in consonance with DOLE D.O.NO. 13 and the OSH Standards must be carried out
on every project

to achieve these objective, we are committed to perform the project.

1. Identifying potential hazards that may arise from the program work activities.
2. Organizing the work activities so as to minimize the risk arising from them.
3. Developing and implementing project specific Safety Health Program
4. Establishing and enforcing all necessary safe work procedure, rules and regulations.
5. Ensuring that all workers are given orientation briefing or induction prior to development to
the site.
6. Establishing a Safety Health Program Committee to act as policy making body of the
project on all issues pertaining to safety and health.
7. Ensuring the conduct of daily toolbox meeting and documents each proceeding.
8. Establishing a system of follow-up of actions and periodic assessment to check program
effectiveness.

f. EMERGENCY OCCUPATIONAL HEALTH PERSONNEL AND FACILITIES

To ensure that the Construction Safety and Health Program is duly followed and
enforced at the Construction Project site, the Contractor should have a minimum required Safety
Personnel to Monitor and inspect the health and safety related aspects of the construction work being
undertaken, the contractor shall provide competent emergency health personnel with the worksite
duly completed by adequate medical supplies, equipment and facilities, based on the total number of
workers, if the contractor only provide a first aid. Clinic or a treatment room he shall provide the
workers in case of emergency, access to the nearest medical or dental clinic. Such access shall the
necessary transportation facilities. In addition, the contractor shall always have in the construction
site required minimum inventory of medicines supplies and equipment in case of emergency.
For long range project, the health and hygiene also imposed by the Safety Personnel this
includes the following:

a. Medical examination for all workers at least one a year


b. Persons with high blood pressure should measure their blood pressure from time to
time
c. Advice the workers regarding the cure on normal sickness like diarrhea cold of flu.
d. Orient the workers about food poisoning

g. SAFETY HEALTH PROMOTION AND EDUCATION

Objectives

Safety knowledge, skill and habits and habits at all levels of the project workplace shall be
promoted maintain and enhanced by regular and continuing safety training and education.

Safety Orientation

New workers will be required to attend a Safety Orientation course before deployment for work.
Upon completion of the orientation, a new worker will be issued with a site ID card gate pass and
Safety Stacker for his hard hat. He will then be required to sign a safety orientation form.

Induction seminar of workers will also include the company safety and health policy, project
clinic procedures, and the company code of employee discipline. The seminar will be participated by
the project nurse or first-aider and the project administrative officer on this representative.

The occurrence of minor injuries at the constructions site is extremely high.
For minor injuries, an adequate first aid kit will allow the injured person or co-workers
to administer first aid and get employee back to work. For major injuries, the contents
of a contractions first aid can be used to provide first aid care to the injured person,
before an EMT or ambulance arrives. It could mean a matter of life and death. The
emergency kits is placed in a cool and dry place along with a fire extinguisher. A
construction site safety survey is always conducted to ensure that the proper first aid
supplies are available for the types of injuries that may occur at the time jobsite.

PORTING INVESTIGATION AND RECORDINGKEEPING REQUIREMENTS

An INCEDENT REPORT is not part of the patients Chart, buy it may be used later in litigation. A report
has two functions:

1. It forms the administration of the incident so management can prevent similar incidents in the future.
2. It alerts administration and the facility insurance company to a potential claim and the need for
investigation

ACCIDENT INVESTIGATION- there are many legal and economic reason for investigation
accident. The result of any investigation should however be used to help prevent a recurrence by
indicating modifications to procedures, premises, training or supervision. All accident should be
investigating, study of incident producing minor injuries or near misses can often reveal.

THE INVESTIGATION-The Officer should look at (a) action taken to prevent a recurrence (b) the
cause of injuries (c) the chain of events leading to the accident, thus allowing the main contributory
factor to be identified (d) whether there was a breach of any statutory requirements under the
health and safety at work.

ACCIDENT RECORDING- a record of an accident must be made in the accident book, B1510
Persons who are covered by the Social Security Act may use the B1510 to give notice to their
employer of an accident resulting in injury. Persons who are not covered by the act have similar
rights under procedures in being able to enter the details of injuries.

RECORDING ACCIDENTS TO STAFF- the injured employee or someone acting on their behalf,
must complete an accident record as soon as they can. Completed accident records should be
detached from this book, and passed to the health and safety adviser. As soon as an entry is made
in accident book it should it should be taken to the office of manager who will make agreements.

RECORDING KEEPING- A record must be made and kept of ail reportable injuries and dangerous
occurrence. The record must contain in each case (a) the date and time of the accident causing the
injury of the dangerous occurrence (b) the following particulars about the person affective. Full
name. occupation nature of injury or conditions
