REF: SECT 8(2)(d) – OCCUPATIONAL HEALTH AND SAFETY ACT (ACT 85 of 1993)

In terms of the above-mentioned Act and Regulations, I hereby appoint you in your capacity as

You are to ensure that daily inspections on this equipment are done and any deviations are reported to me

equipment without written permission from the responsible manager.
Note: These vehicles are to be used for the sole purpose of official company business.
