I, proprietor / partner / director of having
its registered office/office at who is a
registered Local Cable Operator (LCO) having a valid registration to operate in areas of
, have signed an Interconnection Agreement dated
with Siti Networks Limited (hereinafter referred as SITI) hereby
undertake to comply with the following terms and conditions further to the interconnection
agreement signed by me with SITI for the agreed area:
1. That I have signed LCO interconnect agreement with SITI and have received duly signed
copy of the same for my records.
2. That I have received number of Subscriber Application Forms (hereinafter
referred as SAF) from SITI to be duly filled by the subscriber.
3. That I will be wholly responsible to have the said SAF filled up (from the subscribers) in
all the necessary true details of the subscriber on such SAF as per the mandate of TRAI
Regulations on behalf of SITI after due verification and will be responsible for any acts
or omissions on the part of my representatives/ agents in this regard.
4. That after fulfilling all necessary details record in the SAF, the same will be retained by
me for the purpose of record under my custody on behalf of SITI and would be given to
SITI on demand and on expiry/termination of Interconnection Agreemnet.
5. That the said SAF will be produced before such competent statutory authorities and SITI
as and when required for audit and for any other official purposes.
6. That I have received Set Top Boxes (hereinafter referred as STB) from SITI to
be delivered / installed at the subscriber premises.
7. That I will be wholly responsible for the safe custody of such STB’s received by me from
SITI and installed at the premises of the Subscriber, and till the time such STB’s are not
installed at the subscriber premises and written acknowledgment has been taken from
subscribers, they will remain in my possession under safe custody.

8. That any loss / damage in STB’s or any other equipment which will be under my custody
will be my sole responsibility and I will compensate SITI for any such losses resulting
from such loss / damage.
9. That I shall return all the STBs (whether in my custody or in the custody of Subscribers)
in good working conditions in the following circumstances :
a. Termination of the Interconnection Agreement
b. Expiry of the Interconnection Agreement
c. On receipt of demand/advise/instruction by SITI
d. Ceasing of my relationship with SITI
10. I shall hold all the material including STBs in trust for SITI
11. That in case I fail to return the STB’s/hardware received from SITI in good working
condition, I will compensate SITI to the original cost of the STB’s/ hardware.
12. That I understand that SAF is required for validation of subscriber details filled/provided
by me for activation of STB. SITI will have authority to de-activate such boxes if SAF is
not found as per subscriber details updated/provided to SITI from time to time, at the sole
discretion of SITI.
13. That I am fully aware of the functions, features of OYC and the information provided to
me with respect to invoices, collection and outstanding or otherwise through OYC shall
be deemed to be accepted by me unless the same is disputed within 7 days of uploading
thereof.
14. That I will be responsible for the collection of the subscription charges form the
subscribers and issue receipts to them on behalf of SITI, if any and deposit the same with
SITI immediately as per the mutual agreement. Further I undertake to fill up all the
details of subscribers and subscription fee received from them in OYC.
15. That I will be solely responsible to provide SITI all details relating to subscription
amount/STB’s taken from subscribers.
16. That I will abide by all the terms, conditions and policies of SITI and also various rules
and regulations framed by TRAI from time to time.
