IN THE REPUBLIC OF KENYA

AFFIDAVIT
I DENNIS KIOKO MUTHOKA MUMO of Kathiani Sub-County in Machakos County within
the Republic of Kenya do hereby swear this affidavit and state as follows:

1. That I am a male Kenyan citizen by birth.


2. That I was born in the year 2005 as indicated in my birth certificate which reflects my (2)
two names as DENNIS MUTHOKA.
3. That the name KIOKO is my second Surname which I inherited from my maternal great
Grandfather and it appears in all my educational documents.
4. That the name DENNIS is my legal first name and it is recorded in all my documents.
5. That the name MUMO is my father’s name and it appears in all my educational records
as my last name.
6. The (4) four names DENNIS KIOKO MUTHOKA MUMO are legally mine and they
refer to one (1) person by virtue of birth right and subsequent Kamba customary child
naming.
7. That I make this affidavit in request to the Kenya Identity Cards Registration Authority
and any other concerned persons to confirm that I am legally known by (4) Four names as
DENNIS KIOKO MUTHOKA MUMO and prefers to be addressed under my four
names.
8. That I further make this affidavit in support of my application for a Kenyan Identity Card
registration under my (4) four name; DENNIS KIOKO MUTHOKA MUMO to match
uniformly in all my documents for all practical purposes. My birth certificate and all my
educational documents photocopies are hereby attached.
9. That all what is stated herein above is true to the best of my knowledge and belief.
