*This form must be submitted along with your visa application documents.

Dear visa applicants,

Please complete the form below and sign it in the presence of the bank staff, who will then sign
and stamp your bank statements. Also, request the relevant bank staff to endorse this form along
with your bank statements. Your visa application may not be processed until all required details
are verified.

Note: If the bank refuses to sign this form, you must still submit the consent form without the bank
stamp. In this case, please obtain an official account verification letter from the bank.
