Abstract
Attention Deficit Hyperactivity Disorder (ADHD) is a neurodevelopmental disorder characterised by inattention and/or
hyperactivity-impulsivity, lasting for at least six months, and resulting in impaired social, academic, and occupational functioning.
This case report concerns a 30-year-old female with ADHD-related concentration issues. The Boenninghausen Generalities
Method and polarity analysis were used to categorise symptoms, followed by differentiation of medicines to determine the
most indicated remedy, Phosphorus 200C.

Discussion
Categorisation of symptoms
Table 1. Totality of symptoms and polar symptoms,
Chief complaint: Concentration issues, self-reported diagnosis of ADHD based on clinical psychologist’s assessment
of DSM-5.
Location Temples (i.e. temporal lobe located beneath temples of brain)
Sensation • Anxious feeling in body (i.e. butterflies in stomach)
• Choking pain (i.e. choked up in throat when uncomfortable)
• Irritable (i.e. agitated)
• Ill humour (i.e. uncontrollable laughter when uncomfortable/upset)
• Inattentiveness (i.e. forgetfulness mid-sentence)
Modalities • < physical exertion
• << thinking of it (i.e. rumination/mental effort)
• > after vomiting
• >> after sleep
• >> food (i.e. carbohydrate foods like bread)
Concomitants • Frequent urination
o Location: urinary tract
o Sensation: every half hour, or hour
o Modality: < water, > urination
• Shoulder pain
o Location: upper shoulder joint
o Sensation: dull throbbing
o Modality: < intense concentration (eyes strained, reading), > muscle relaxation
• Vivid dreams
o Location: limbic system
o Sensation: violent (e.g. alien attack, tower crash)
o Modality:, < stress, > security & safety (housing)
• Pungent perspiration
o Location: armpits
o Sensation: smelly
o Modality: < stress, > deodorant & movement
Symptoms are selected with emphasis on sensation and modalities, as these symptoms, particularly the polar symptoms,
form the basis of the patient as a whole and indicate a more suitable remedy that matches the characteristic symptoms of
the patient, provided there are no contraindications.13
Rubrics
Firstly, modalities - circumstances and conditions that worsen or improve a complaint, are considered with aggravations
noted first, then ameliorations.18
Figure 1. Modalities

Secondly, sensations - perceived experiences of a complaint, are considered, noting that 3 are mental sensations and
ranked last. There are no polar symptoms so the non-polar symptoms are:
Figure 2. Sensations
Physical generals - specific parts of the body and/or organs, including location are considered, this is non-polar.
Figure 3. Physical generals, location
Thirdly, concomitants - secondary complaints that occur during or around the same time as the main complaint, are
considered with polar symptoms noted first.18
Figure 4. Concomitants, polar.
Then concomitants that are non-polar are considered.

Lastly, mentals - perceived experiences of a complaint pertaining to the mind or process of thinking, are considered for
reference and differentiation, with polar symptoms noted first.18

Then mentals that are non-polar are considered.

Repertorisation
Before PA, the repertorial matrix indicated remedies in order of suitability according to the individual’s presenting symptoms with
Pulsatilla listed first, reflecting 11 of the 15 symptoms and a total score of 29, followed by Nux-Vomica listed second, reflecting
10 of the 15 symptoms and a total score of 28. With modalities and sensations prioritised, symptoms 1 to 7, Nux-Vomica would be
more suitable with a total score of 15 versus Pulsatilla with a total score of 13 however, further examination of the remedy’s specific
indications and individual’s overall presentation is required following PA to determine the preferred remedy.
Figure 8. Repertorial Matrix, before PA.

Case study Australian Journal of Herbal and Naturopathic Medicine 2024 36(2)
After PA, the repertorial matrix was updated to a more reliable and precise list indicating remedies in order of their sum
of symptoms as well as their polarity difference. With the latter prioritised, Natrium Carbonicum and Baryta carbonica
are considered the most suitable remedies both with a polarity difference of 9, followed by Phosphorus and Coffea cruda
which both have a polarity difference of 7. Notably with PA, Pulsatilla is then considered the least suitable remedy as 5
of the 7 polar symptoms are weighted strongly in opposition and therefore contraindicated. Further examination of the
remedies with a higher polarity difference provided a means for prescription differentiation.
