JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

That we, LORNA R. BAEL AND LORETA B. BAGALANON, both legal age, Filipinos and
are residents of Purok Mangga Banonong (Pob), Dapitan City, respectively, after having been
duly sworn to an oath in accordance with law, depose and say;
That we personally know LOVELY NADETTE G. BAYBAY, being the daughter of
Agilberto C. Gallosa and Bernaditha C. Gallosa;
That said LOVELY NADETTE G. BAYBAY was a solo parent since December 2010 until
this present;
That we are executing this joint affidavit in order to attest to the truth of the
aforementioned facts and circumstances and for whatever legal purpose it may serve.

That I, LOVELY NADETTE G. BAYBAY, of legal age, Filipino, married and a resident of
Teacher’s Village, Banonong, Dapitan City, after having been duly sworn to an oath in
accordance with law depose and say;
That I am an applicant for Solo Parent at the City Social Welfare and Development
Office Dapitan City;
That since December 2010 I have gave birth to my child by the name of Lark Matthew
G. Baybay, and I am supporting him as my legitimate child;
That I am executing this affidavit in order to declare that since January 2017 until this
present I am a solo parent and I am going to support my application for solo parent.
