I, the undersigned parent/guardian, hereby give permission for my child,


____________________, a student athlete of Isio Private Academy, Inc., to
participate in the basketball practice game on November 11, 2024 at Galicia Baptist
Church, Goce, Ilog, Negros Occidental, in preparation for the upcoming municipal
meet. I understand that my child will be under the supervision of school staff and I
acknowledge the potential risks involved in sports activities.
I release and hold harmless the school, its staff, and all involved parties from any
liability that may result from my child's participation.
