AFFIDAVIT OF LOSS

I, JOAN C. ASINGUA, of legal age, Filipino and with residence


and postal address at M117 Arellano St., Zone V, Iba, Zambales, being
first duly sworn in accordance with law, depose and state that:

1. I am a registered driver in Land Transportation Office


with Driver’s License No. Driver’s License No. C06-10-
000851;

2. However, recently, I noticed that my Driver’s License


was missing from my wallet when I checked my
belongings;

3. I searched high and low for my Driver’s License but still


could not locate the same;

4. I hereby expressly state that the aforementioned


Driver’s License can now be considered, for all legal
intents and purposes, lost beyond recovery; and

5. I am executing this affidavit to attest to the truth and


veracity of the foregoing facts.

2022 at Iba, Zambales. Affiant exhibited to me her Philippine National
ID No. 2763-9038-5216-5743 and is known to me to be the same
