Subject: Investigation Report from Customer Concern form: 009

This is related to the customer concern form submitted January 14, 2025, with
reference number 009, Mr. Dominic C. Tambiga, the affected customer, reported
the incident that occurred earlier that day at 9:00AM in Brgy. Bagumbayan, Teresa,
Rizal.
According to Mr. Tambiga's statement, the incident occurred while our maintenance
team’s leak repair activity in the vicinity of his business. Mr. Ryan Olayres, who is a
member of the maintenance team assigned to this particular task, was responsible
for carrying out the repairs. Unfortunately, during this process, there was an
unforeseen increase in water pressure, which led to water accidentally flooding
several sacks of rice stored at Mr. Tambiga's facility.

After conducting a thorough investigation, Mr. Tambiga has expressed that he does
not seek compensation at this time. He acknowledged that the situation was an
accident and appreciates the efforts of the maintenance team. He also added that
he just wants to inform the water district about the incident because this is the 3 rd
time that leak repair activity happened in that area and no restoration work has
done. Later that day, the management instructed the maintenance team to return
to that area to carry out restoration work. The restoration has been completed, and
the pavement is currently curing.

However, he has provided a constructive suggestion for future operations. He


recommended that prior to conducting any leak repair activities, the team should
implement adequate warning devices and utilize all necessary tools and equipment
to lessen the risk of similar incidents that might happen in the future.

Additionally, Mr. Tambiga indicated that if the soaked rice is unfit for sale as a result
of the incident, he will promptly reach out to the district to discuss the possibility of
compensation for the damages incurred. Mr. Tambiga is willing to cooperate as we
work to address this issue.
