Subject: Authorization to Claim Scholarship Pay-Out

Dear Sir/Madam,

I, Mark Jefferson C. Luna, a beneficiary of the KAFUERTE Scholarship Program, I’m


unable to personally attend the scheduled Scholarship Pay-Out on March 21, 2025, due to my
class commitments.

In this regard, I hereby authorize my mother, Julie Luna, to claim my scholarship pay-out on my
behalf. She will be presenting her valid ID along with a photocopy of my school ID for
verification purposes.

I understand that my representative must comply with all requirements set by your office to
successfully claim my financial assistance.
