PARENTAL CONSENT

We, hereby willingly and voluntarily give consent to the participation of


my son _____________________________ in the SUMMER BASKETBALL
TOURNAMENT on the month of May 2025 at Cataingan Municipal Astrodome,
Poblacion, Cataingan, Masbate.

We have considered the benefits that our son will derive from his
participation in this activity provided that due care and precaution will be
observed to ensure the safety of our son, and that organizers and other persons
involved may not be held responsible for any untoward incident that may happen
beyond their control.
