This letter serves to confirm the registration of Mr Reduan Patwary, with date of birth 13th May
2004, residing at Borkotpur Nabiganj, Nabiganj, Nabiganj, Habiganj, Sylhet, at our clinic.

Mr Reduan Patwary has been registered at our clinic since 2006. His initial registration was
completed using his paper birth certificate. Subsequently, our records were updated on 03/09/2022
with his online birth certificate issued on 11/03/2022.

Additionally, it is confirmed that Mr. Ibrahim Patwary and Mrs Sana Begum are documented as his
parents and next of kin in our records.

Mr. Ibrahim Patwary has paid for all expenses related to Reduan Patwarys medical treatments and
