Re: Mr David Taylor, Aged: 38

I am writing to refer Mr Taylor who is worried and unable to do full time work due to knee joint pain
and swelling for further assessment and management.

Mr taylor is a father of three children and work as a landscape gardener, actively involved in sports
activities. He smoke 1 pack per day and drinks 1-2 beer and more on weekends. Past medical history
shows left inguinal hernia two years back.

On 12/08/10 Mr taylor presented with complain of knee joint pain and swelling which limited his work
activity due pain, Due to twisting of knee joint in tennis 6 months ago. On examination his knee joint
were swollen and tender on medial aspect of the knee.This has limited his activity to work and he is
worried about his income. Patient advised to take voltarin 50mg bid, reduce smoking and visit if there is
no improvements.

Subsequently Mr Taylor reviewed for growing symptoms of intermittent pain and swelling which has
affected his work and his income. On examination patient has a swelling, tenderness on his left knee and
decreased range of motion. Assessment shows medial cartilage injury and ordered MRI.

On today visit patient symptoms has not changed from previous visits and MRI shows medial cartilage
damage.

It would be highly appreciated if you could further evaluate and manage Mr Taylor condition at your
earliest convenience.
