Description Of Operation

Skin Preparation : BETADINE
Incision : SKIN - SUBMENTAL & SUBMANDIBULAR
Findings : SPACE INFECTION
Procedure : After securing adequate general anaesthesia via nasal intubation with fibre-optic
tube. Patient was positioned , painted & draped. Inferior border of the mandible was
marked. Transverse Incision is marked in the skin below the symphysis of the
mandible. Blunt dissection is carried out by inserting sinus forceps through this
incision upwards & backwards. Next an incision is marked 2 cm below the inferior
border of the mandible . Skin & subcutaneous tissue is incised. Sinus forceps is
inserted through the incision superiorly & posteriorly on the lingual side of the
mandible below the mylohyoid to release pus from submandibular space. Left
parapharyngeal oedema was noted & intra-oral incision [sicher’s incision] is made
vertically over the lower part of the anterior border of the ramus of the mandible deep
to the bone. Sinus forceps was passed along the lateral surface of the ramus
downwards & backwards near pterygomandibular space & Pus was drained. Sample
was sent for culture & sensitivity. Extraction was done irt 37,38,48. Jackson's drain
was placed submandibular ,submental extraorally and submassteric intraorally placed
and closure was done. Patient was reversed from general anaesthesia & shifted to
post-op ICU with ET Tube for observation & monitoring of vitals.
