Waiver of Liability

This agreement releases Alfamart Trading Philippines, Inc. from all liability relating
to injuries that may occur during site visits and any related work outside the Head Office.

By signing this agreement, I agree to hold Alfamart Trading Philippines, Inc. entirely
free from any liability, including financial responsibility for injuries incurred, regardless of
whether injuries are caused by negligence.

I also acknowledge the risks involved in the above mentioned activity/ies.

I swear that I am participating voluntarily, and that all risks have been made clear to me.
Additionally, I do not have any conditions that will increase my likelihood of experiencing
injuries while engaging in all related activities to our site visits.

By signing below I forfeit all right to bring a suit against Alfamart Trading Philippines,
Inc. for any reason. In return, I will gain experiences and learnings. I will also make
every effort to obey safety precautions as explained to me verbally. I will ask for
clarification when needed.

Lastly, IN SIGNING THIS WAIVER, I ACKNOWLEDGE AND REPRESENT THAT I have


read and foregoing Waiver of Liability, understand it and sign it voluntarily as my own
free act and deed; no oral representations, statements, or inducements, apart from
foregoing written agreement, have been made;
