NOTE: DATE OF BIRTH ON CV, EMPLOYMENT FORM, MSL FORM,

BIRTH CERTIFICATE, BVN PRINT OUT FROM YOUR BANK, WAEC…


DAY, MONTH AND YEAR ON ALL MUST BE THE SAME.
You are to fill the form with black pen. All lines must be well filled, you
need two guarantors to fill the guarantor page 3 Please nominate two
persons who are blood relatives as guarantor. The individual must be
above 30 years old, is fully resident in Nigeria and is employed in an
organization that has more than 20 full-time employees or is a business
owner with at least five employees.,
 Your guarantor 1 will fill fidelity guarantee page 5
 Your guarantor 2 will fill fidelity guarantee page 6
 Attached photocopy of the two guarantors valid ID card to Fidelity
guarantee page 5&6 with their passport photograph.
 You also need two recommendation letters from two referees
different from your guarantor, see the address the letter will be
directed to below:

Km 19, Lekki Epe Express Way,
Lekki Lagos.
6 Passports in white background
One passport on the front page of application form
Two passports on mortgage form
One passport on ID card form.
Two passports on MSL.
One full picture corporately dress
The following should be attached together in this order:
CV
NYSC For graduate
B.Sc./HND/OND
SSCE
Birth Certificate/declaration of age
Medical fitness report for male
Medical fitness and pregnancy test for female.
Reference letter from two referees (Different from your guarantors)
BVN print out from your bank (Take note of date of birth on it)
NIN
Photocopy of pension form or pension pin
Criminal Background form.
