Requirements for Filing a Report of Adoption (ROA)
For processing of a Report of Adoption, please accomplish and submit the following:
1. Four (4) copies of the Form for Report of Adoption (ROA). This form is available at
the Embassy or may be downloaded from the Embassy’s official website at
www.philembassy-uk.org
2. Each ROA form must be accomplished and duly signed by applicant
3. ROA form may only be filed by the adopting parent/s and duly signed by
him/her/them.
4. Original document and four (4) copies of the Final Decree of Adoption duly- issued
by the pertinent Court/Authority in the United Kingdom. The original copy of the
decree will be returned to the applicant.
5. Four (4) copies of the data page of the passports of the Adopting Parent, other
parent (Mother/Father) as the case may be, and the adopted child.
6. Payment of £ 18 in cash or postal money order made out to the Philippine
Embassy
The Report of Adoption will be released to the applicant on the next working day after
filing.

This is to certify that this Embassy has accepted, for registration purposes, the Adoption Decree of

This application for Report of Adoption, together with its enclosures, were submitted/filed by
The person whose signature appears below and who personally appeared at the Embassy for
purposes of notification and registration of the Report of Adoption in the Philippine Civil Registry.
This Certificate/Report shall be forwarded to the National Statistics Office, Manila, Philippines,
through the Department of Foreign Affairs, for recording and notation on the Birth Certificate of the
adopted child.
