During the Server Training presentation, fill in the blanks and take notes. Keep the worksheet as
reference and use it to train other employees at your liquor establishment.

LIQUOR LAWS of HAWAII & RULES of the LIQUOR COMMISSION

● Valid, unexpired Government issued photo ID (Driver’s License, State ID,
Passport, or Military ID) – No digital or printed copy accepted
● Social Security Card, or printed W-2 form, or printed paystub with legal
name and at least 4-digits of your SSN (for 1st time registration with the
Liquor Commission)
● Manager’s Authorization Letter (for Manager/Assistant Manager positions only)
● $10 (cash or credit/debit card)
● Email confirming passing server training (digital copy accepted).
