AFFIDAVIT OF LOSS
I, CHRISTOPHER ARENOS, representative of PEOPLE CARGO TRANSPORT
CORPORTION, of legal age, Filipino, with business address at #41 Villa Maria St.
Sangandaan Caloocan City, after having been duly sworn to in accordance with law,
do hereby depose and say, THAT:
1. That I am OWNER/OPERATOR of the vehicle described as follows:
MAKE/TYPE : ISUZU
MOTOR No. : 449262
CHASSIS No. : HMW4000C88
PLATE No. : DFY 978
Around Dec. 28 - 31, 2024 during holyweek, my vehicle was properly parked in my
parking area at Caloocan City, when I returned to retrieve the above-described
vehicle I noticed that the front (1) and back (1) plate of my vehicle is missing;
3. I lost no time in diligently trying to locate the said plate/s but all efforts
exerted proved futile and to no avail and that the same is considered lost or
stolen and beyond recovery:
4. That said lost plate/s was/were never lent to another person or entity
nor confiscated by any enforcement agency:
5. I bind myself to surrender the said plate/s to the district office that
issued the same in the event that said plate/s are recovered;
6. If the DOTC thru the LTO/LTFRB, after inquiry, has found me responsible
for the unauthorized use of the herein reported lost plates, I hereby signify
my conformity to be meted the administrative fines and penalties due and
imposed thereon, including the grounding of the units covered by the
certificate
of Public Convenience issued therefore, with waiver and desistance on my
part
for any claim or action by reason of the aforesaid positive measure taken
against
me.
7. Purpose of this affidavit of loss;

8. I am the executing this affidavit for the aforesaid purposes as well as of

ORIGINAL OFFICIAL RECEIPT (OR)
I, BALJINDER KAUR, of legal age, Indian nationality, with
residence address at 119 K 1ST KAMUNING, QUEZON CITY, after having
been duly sworn to in accordance with law, do hereby depose and say,
THAT:
1. That sometime on December 2016, the Original Official Receipt
of my Non-Professional Driver’s License was inadvertently lost
and nowhere to be found at the abovementioned address
presumably the same was misplaced;
2. That I exerted diligent efforts to locate said Original Official
Receipt of my Non-Professional Driver’s License, but proved futile
hence I concluded that it is beyond recovery and irretrievably
lost;
3. That for all intents and purposes, the aforesaid documents
amounted are now considered lost;
4. That I am executing this affidavit to attest to the truth of the lost
of my above mentioned Original Official Receipt of my Non-
Professional Driver’s License for CLAIMING the ID card of my
Non-Professional Driver’s license and for whatever legal purposes
and intents it may serve.
