I, FE LUNOP PALMA, of legal age, Filipino, widower and a
resident of Purok Maligaya 3, Tanabog, Tandag City, Surigao del
Sur, state under oath, THAT:

1. I am the only sole-surviving heir of my late mother


VERONICA ESTAÑO LUNOP, who died intestate without
any debt.

2. My mother is a beneficiary of UNCONDITIONAL CASH


TRANSFER (UCT) program by Department of Social
Welfare and Development (DSWD).

3. My mother is the holder of the OFFICE OF THE SENIOR


CITIZENS AFFAIRS (OSCA) ID NO.__________ issued by
Municipality of Cagwait, Surigao del Sur.

4. That, I kept my mother’s personal belonging including her


OSCA ID NO. ___________, in my cabinet.

5. However, when I search the said OSCA ID No. _____, it was


lost and can no longer be found despite diligent efforts to
locate;

6. I exerted all efforts to locate the same but to no avail;

7. I am executing this Affidavit to apprise on the fact of said


loss and for whatever legal purpose this may serve best.
