1 OBJECTIVE
This plan sets up minimum requirements to be in place, which will help the rescue process of
the entrants who are in a situation which is dangerous to their life and health. It outlines the
necessary effective arrangements to carry out the rescue operation in an emergency and what
are the general requirements for the rescue operation before one makes a direct entry into a
confined space.

2 PURPOSE
The purpose of preparation of this plan is to manage emergencies in a confined space, for
ensuring personnel entering in the confined space can be assured of their safety during
emergency conditions.

3 SCOPE
This document outlines the rescue operations to be conducted for confined space entrants and
the procedure and lines of communication that need to follow. This will be applicable for the
emergency teams which will be stationed for specific confined spaces and will outline the roles
and responsibilities and the plan for rescuing entrants in a confined space.

4 DEFINITIONS
• Confined Space – Any space that has limited or restricted means of entry or exit. Is not
designed for human occupancy. Contains or has the potential to contain a hazardous
atmosphere. Contains any other recognized serious safety or health hazard(s). Excavation/
trench deeper than 1.2m shall be considered as confined space.
• Confined Space Entry Supervisor – A work permit issuer, or an individual who has been
assigned by an issuer, who is responsible for directing all aspects of the confined space entry.
Confined space entry supervisors shall be trained and qualified to carry out their responsibilities
per instruction.
• Confined Space Stand-by man- Individual(s) assigned by the confined space entry
supervisor at each designated entry point to continuously monitor the confined space entry
while personnel (entrants) are inside the confined space. Confined space entry standby men
shall be trained and qualified to carry out their responsibilities per the instruction.
• Work Permit – it is a written authorization obtained by contractor from the client to

perform the job. It is also the record of work and direct communication between job supervisor
and issuing Supervisor. It specifies job activity, hazards, safety precaution and rules to perform
job safely. It specifies job activity, its hazard and their precautions.
• Work Permit Issuers – Supervisor who are certified by their division or department to
issue work permits for restricted areas under their supervision (also referred to as issuers).
• Work Permit Receivers – Craft supervisors, craftsmen, or other company employees
who have been certified by their division or department heads, and contractor employees
sponsored by organization heads to sign and receive work permits (also referred to as
receivers).

5 GENERAL REQUIREMENTS
The following are the general requirements to be fulfilled with regards to the emergency
arrangements needed for a rescue operation:
 Appropriate rescue and resuscitation equipment: Gas Monitor, Full-Body harness, Rope
and Carabineers along with a First Aid Kit.
 Specific alarms for specific types of emergencies: The emergency can be communicated
in a number of ways for example by the tug of a rope, by radio or by means of a ‘lone
worker’ alarm.
 Safeguarding the rescuers: Multiple fatalities have occurred when rescuers have been
overcome by the same conditions that have affected the people they have tried to
rescue. To prevent this, those who have been assigned a rescue role, for example
members of an in-house or works rescue team, must be aware of the risks within the
space and protected against the cause of the emergency.
 Fire Safety Precautions: When there is a risk of a fire, appropriate fire extinguishers may
need to be kept in the confined space at the entry point. In some situations, a sprinkler
system may be appropriate. In the event of a fire, the local fire service should be called
in case of the fire cannot be contained or extinguished. Care is needed when deciding
whether or not the ventilation system should be kept working or switched off because
either course may affect the chances of escape or rescue. Continued use of ventilation
system may also affect the development of the fire, because forced air may fan the
flames.
 Shut-Down Procedures: There may be a need to shut down adjacent or nearby plant
before attempting an emergency rescue, either because the plant could be the cause of
the emergency or safe entry cannot be gained without the plant being shut down.

 Medical Aid: Appropriate first-aid equipment should be provided and available for
emergencies and to provide first-aid until professional medical help arrives. First-aiders
should be trained in its use and to deal with the foreseeable injuries.
 Appropriate Trainings and Qualifications of the Rescue Personnel: Those likely to be
involved in any emergency rescue should be trained for that purpose. The training needs
for each individual will vary according to their designated role. Refresher training should
be organized and available on a regular basis, for example annually.

6 RESPONSIBILITIES
6.1 PROJECT MANAGER:
 Will be responsible to identify clear roles and responsibilities with respect to confined
space entry.
 Will be responsible to identify the work sites according to Site Condition, the Work
Permit Requirements that which permit will be valid for the work.
 Will be committed in getting the employees trained and make necessary
arrangements for special rescue operation trainings.
 It will also be his responsibility to provide the necessary emergency
equipment, appropriate signage and personal protective equipment, specific
for the job.

6.2 HSE MANAGER:


 Will be responsible for acknowledging the specific confined space trainings and
rescue operation trainings along with other trainings like gas tester’s training to be
done.
 Will be responsible to implement the Confined Space Entry plan and this Rescue Plan
on sites exposed to dangers of confined space.

6.3 CONSTRUCTION MANAGER


The construction personnel will make sure that:
 Their employees are appropriately trained
 The execution of work is done in a safe manner i.e., follows the approved plans.
 They identify the PPE needs on site, the emergency equipment needed, barricades and
signage to be posted, necessary documentation i.e., Work Permit, HIP, MOS, JSA, etc.
are in place where there is a confined space entry being made.

6.4 CONFINED SPACE ENTRY SUPERVISOR


 Is responsible for all activities to be conducted in the confined space.

 Arranges the specific documents needed for on-site work i.e., Work Permit, JSA,
Confined Space Entry Checklists, Gas Test Logs, etc.
 Monitors the activities for safe execution and identifies the safety requirements are in
place.
 Arranges the emergency equipment and coordinates with the Standby Man and
the Rescue Team in an emergency.

6.5 HSE OFFICER


 The HSE Officer on site will be responsible for identifying the hazards associated with
the execution of work in confined spaces and recognizing the means to eliminate or
reduce the hazards and communicate the hazards and their controls to the construction
personnel their HSE Supervisor.
 The HSE Officer will be responsible to implement the Job Safety Analysis for the specific
activity and ensure the Confined Space Entry Plan and Rescue plan are being complied
with.

6.6 CONFINED SPACE ATTENDANT (HOLE WATCH/STANDBY MAN)


 Prevents unauthorized access into the confined space.
 Ensures the well-being of entrants through constant communication and identifies signs
of emergency situations.
 Ensures the entrants are fully equipped with the necessary protective equipment prior
to entry.
 Maintains an entry log of the entrants through a Time-In and Time-Out Log Sheet.
 Responsible to communicate with the supervisor for tools/materials needed in the
confined space and also in cases of emergency.

6.7 GAS TESTER


 The gas tester at site must remain on site at all times and continuously monitor the
fluctuating levels of Oxygen, Carbon Monoxide, Flammable Gases and H2S.
 Maintains a log every 2 hours and records the findings.
 If the findings are beyond the acceptable limit, informs the supervisor immediately.

6.8 RESCUE PERSONNEL


 The rescue team is designated by the management with the necessary trainings and the
equipment provided.
 The rescue team is the active emergency response team in case of emergencies in
confined spaces and executes the rescue procedure when needed.

6.9 CONFINED SPACE ENTRANT(S)

 The entrants must be oriented for safety issues on site and specifically attend the
confined space safety awareness training session.
 The entrants must ensure the necessary equipment needed in the confined space and
the personal protective equipment is complete and in the appropriate numbers, and in
the case of PPE the fitting is appropriate.
 The entrants must sign in the log sheet while entering the confined space maintained by
the Confined Space Attendant/Standby Man and sign themselves out as well, when
exiting.
 The entrants must attend an emergency response training and should be well aware of
the emergency escape routes and procedures in the case of one.
 The facial features and other factors that affect the efficiency of RPE must be well-
maintained and groomed by entrants.

7 CONFINED SPACE ENTRY PLAN


Confined space entry plan is prepared with this document according to the Construction Safety of
China Railway No: 3 Engineering Company at the Sapphire Riyadh Factory Project.

8 ATMOSPHERIC GAS TESTING


 Atmospheric gas testing within confined spaces shall include, but not be limited
to, testing for oxygen atmospheric gas testing within confined spaces shall
include, but not limited to, testing for oxygen (O2), flammable mixtures (Lower
Explosive Limit [LEL]) toxic gases (e.g., H2S), and carbon monoxide (CO).
 Atmospheric gas tests shall be performed and immediately recorded:
o Prior to entry.
o After breaks or other interruptions in the work.
o If there is any reason to believe that conditions inside the confined space
have changed.
o At periodic intervals (e.g., every 1 hours) as necessary to determine
whether acceptable atmospheric conditions are being maintained during
the course of the confined space entry
 Atmospheric gas tests shall only be conducted by a SA-certified gas tester except
for grassroots construction projects which shall be per Work Permit System.

9 CLEANING OF CONFINED SPACE ENTRY


1. Initial gas test shall be taken prior to cleaning of confined space.
2. Confined spaces shall be cleaned and decontaminated of hazardous materials.
3. Good housekeeping shall be maintained to avoid tripping hazards.
4. Dewatering shall be done if any water accumulated in confined space.

5. The necessary measure shall be taken to clean the entrants and the equipment during
and after the work in confined spaces.

11 TYPES OF RESCUES TO BE CONDUCTED


11.1 SELF-RESCUE:
An employee trained to recognize a critical condition or symptoms of exposure will exit the space
on his own. Alternatively, a standby man, who is outside of the space, if recognizes a new

hazard will instruct individuals to leave the space before they are affected.

11.2 NON-ENTRY RESCUE:


A qualified person will prohibit the entry of any other personnel and this can be done via a safety line
attached to the personnel in the confined space or by grabbing the personnel with a rope, strap or
pole and pulling them to safety.

11.3 ENTRY RESCUE:


This will be used as a last resort as having more personnel enter an area that has already
incapacitated one or more person’s places the rescuer at a considerable risk. Entry rescues will be carefully
planned and executed to avoid creating more victims in need of rescue. Rescuers will be made be aware of
their surroundings and plans will be reevaluated immediately if there is any change in the conditions of the
confined space.

12 SPECIFIC CONFINED SPACE TRAININGS


The Confined Space Rescue Personnel will be trained for the following:

13 CONFINED SPACE TRAININGS FOR GENERAL PUBLIC


For the general public, a confined space training should be arranged to provide descriptive
guidelines and accurate applications, with current equipment availability and regulatory
influences, for leadership in the construction industry to apply towards activities and practices
in their working environment.

14 APPENDICES
14.1 TACTICAL ASSESSMENT - CHECKLIST FOR CONFINED SPACE RESCUE
To assess an emergency situation for ensuring the rescuer’s safety, the following requirements are
first taken into consideration with the help of a checklist which are as follows

1. Work Permit: An appropriate work permit must be obtained prior to a confined


space entry. The permit must be in place at all times during the work.
2. Standby Man: The standby man monitors the activities and maintains communication

with the entrants. He must be coordinated with during the emergency.


3. Rescue Team: A rescue team must be appointed prior to the entry.
4. Communication: The communication methods must be taken into consideration.
5. Access: An access must be made for access and egress.
6. First Aid: A First aid box must be present on site.
7. Personal Protective Equipment: Appropriate PPE must be worn before entry into the
confined space and must be provided to all entrants.
8. Warning Notices & Barricades: Appropriate warning notices must be posted and
barricades should be in place where there exists a fall hazard or an unauthorized access.
9. Adequate Lighting: There must be adequate lighting in the confined space to prevent
obstructed eye sight due to low levels of visibility.
10. Ambulance and Other Logistics: The ambulance must be patrolling the place or at best
be constantly present for the confined space and other emergency vehicles must be
present as well.
11. Gas Detectors: Gas Detectors and other specialized equipment must be provided.

11.1ON-SITE RESCUE
The On-Site Rescue Plan is part of the Confined Space Entry Permit and is based on
the assessment of hazards in the space.

11.1.1 Prior to entry and/or work in the Confined Space:


 The Entry Permit Supervisor will ensure that the onsite rescue plan for the
confined space has been completed and that all the rescue equipment
identified in the plan is available to effect a rescue in the confined space.
 The Entry Supervisor will ensure that an adequate number of appropriately
trained personnel (as documented in the rescue plan) are available for
immediate implementation of the rescue if so required.
 The Entry Supervisor will ensure that all personnel in the rescue team,
understand and know their roles and responsibilities and have signed the
rescue plan prior to any personnel entering the confined space. Ensure
everyone is aware of the evacuation alarm.
 The Stand by person must establish communication with all workers (inside
and outside of the confined space) using the means described in the rescue
plan.

11.1.2 On entry and while working in the Confined Space


The Stand by person who is stationed outside and near the entrance to the confined space as
shown in the rescue plan, remains in constant communication with all workers inside the
confined space.

 The Stand by person must be notified immediately if an entrant recognizes:


o Unusual action or behavior
o An unexpected hazard
o An unsafe act
o Detects a condition prohibited by the permit
 Personnel must exit the confined space as quickly as possible, when:
o An order to evacuate is given by the Entry Permit Supervisor.
o An entrant recognizes a sign or symptoms of exposure
o An unacceptable condition arises
o An evacuation alarm is activated

11.1.3 In the event of a confined space rescue


The Entry Permit Supervisor or the Stand by person does not enter the confined space but
immediately summons a rescue response from the on-site rescue team. A nominated person must
inform emergency services immediately if required. If injured person is able to be extracted, assess the
person’s injuries and provide assistance and first aid as necessary.

11.2.1 Procedure
On identification of an emergency in a confined space the following procedure will be followed:

11.2.1.1 Project Manager / Deputy Project Manager


 In charge of any emergency response
 Authority to mobilize any other service or personnel as detailed in the “Project
Emergency Response Plan Flow Chart”, to receive information and make sure it
is recorded.
 Appoint log keeper from Administration Department and confirm a control point.
 Notify all concerned when emergency is over and collect reports of incident to HSE
manager for investigation and analysis.
 Identify deputy in their absence and make it known to the Administration Department.
 Review and approve the project emergency plan
 Provide sufficient/material/personnel resources to ensure effective emergency
response is available at any time
 Initiate incident investigation and ensure timely follow up.

11.2.1.2 Incident commander


 Organize the execution and training of the CSRP (Confined Space Rescue Plan)
 Review and update the information
 Monitor and audit the system in line with the HSE audit program.
 Ensure the emergency plan/procedure are in place and coordinated and communicated
to all employees
 Ensure emergency drill/exercises should be conducted and evaluate the awareness of
the emergency plan
 Ensure that emergency trainings and drills are documented, the results
evaluated, improvement actions assigned and their completion is monitored.
 Develop scenarios for drills and conduction schedule.
 Arrange observers for drills and inform company representative about drill and scenarios.
 Evaluate the awareness of each individual during an emergency.

11.2.1.3 Construction Manager


11.2.1.3.1 Familiarize personnel with the CSEP (Confined Space Entry Plan) and CSRP
(Confined Space Rescue Plan).
11.2.1.3.2 Ensure the necessary facilities, equipment and trained personnel are available.
11.2.1.3.3 Ensure the CSRP preparedness is maintained adequately.
11.2.1.3.4 Ensure the site E.R.T. is in place and that key personnel are available through a duty call

out system.
11.2.1.3.5 Ensure the operational and logistical support is available.
11.2.1.3.6 Ensure continuous coverage of the communication system (phone/radio)
during all working hours.
11.2.1.3.7 Coordinate overall emergency response actions.
11.2.1.3.8 Conduct Emergency Response drill with the HSE Manager and the SAPMT HSE Personnel.

11.2.1.4 Emergency Response Team: (E.R.T)


11.2.1.4.1 Attend incident site urgently.
11.2.1.4.2 Follow instructions of Team Leader and Site Manager.
11.2.1.4.3 Coordinate with site manager in the response of a Confined Space Emergency
Credible Scenarios.
11.2.1.4.4 Communicate to the PMT HSE if any emergency occurs.
11.2.1.4.5 Audit the effectiveness and preparedness of the Emergency Response Team.
11.2.1.4.6 Seek advice from Safety Supervisor.
11.2.1.4.7 Organize and control overall performance of the Emergency Response Exercises.
11.2.1.4.8 Ask for back-up if required.
11.2.1.4.9 Be responsible for all required documents and reporting system.
11.2.1.4.10 Make recommendations, if necessary, on any point.

11.2.1.5 HSE Supervisor


11.2.1.5.1 Know and apply the Confined Space Rescue Plan and ensure it is brought to the notice.
11.2.1.5.2 Ensure all staff work in a safe manner and use all protective devices and procedures.
11.2.1.5.3 Advice all employees of any potential or actual dangers and how to isolate,
prevent or remove of them.
11.2.1.5.4 Ensure that new employees understand, and comply with the CSRP.

11.2.1.6 Project Nurse


11.2.1.6.1 Be the lead of emergency medical treatment.
11.2.1.6.2 Advise on medical measures.
11.2.1.6.3 Maintain the medical register & track the injuries.

11.2.1.7 Confined Space Entrants


All personnel shall be responsible for doing as the following:
11.2.1.7.1 Understand and comply with Confined Space Rescue Plan and its courses of action.
11.2.1.7.2 Take every reasonable precaution to protect the safety of other
workers and himself/herself.
11.2.1.7.3 Communicate the emergency situation to the responsible person i.e.
Designated Standby Man or Supervisor whichever is applicable.

11.2.1.7.4 On notification of an emergency, ERT responds to the incident.


11.2.1.7.5 Follow the instructions of the Supervisor on site and the HSE Officer.
11.2.1.7.6 The related departments /authorities will be contacted based on the
requirement of RTCC reporting format as soon as possible, if necessary, if no
back up is required, report emergency is in hand and implemented immediately.
As the regulated procedure, and notify the correct personnel when the potential
case is found.

11.2.2 Responsibilities

11.2.2.1 Project Manager / Deputy Project Manager


 In charge of the confined space emergency response.
 Notify all concerned when emergency is over and collect reports of incident to HSE
manager for investigation and analysis.
 Identify deputy in their absence and make it known to the Administration Department.
 Provide sufficient/material/personnel resources to ensure effective emergency
response is available at any time.
 Verbal notification of the emergency to company representative.
 Initiate incident investigation and ensure timely follow up.

11.2.2.2 Incident commander


1. Organize the execution and training of Confined Space Entry Rescue Procedure.
2. Review and update the information.
3. Ensure emergency drill/exercises should be conducted and evaluate the awareness of
the emergency plan.
4. Ensure that emergency trainings and drills are documented, the results
evaluated, improvement actions assigned and their completion is monitored.
5. Develop scenarios for drills and conduction schedule.
6. Arrange observers for drills and inform company representative about drill and scenarios.
7. Evaluate the awareness of each individual during an emergency.

11.2.2.3 Construction Manager


1. Familiarize personnel with the confined space entry rescue plan.
2. Ensure the necessary facilities, equipment and trained personnel are available.
3. Ensure the CSE Rescue preparedness is maintained adequately.
4. Ensure the site emergency organization is in place and that key personnel are available
through a duty call out system.
5. Ensure the operational and logistical support is available.
6. Coordinate overall emergency response actions.
7. Conduct Emergency Response drill with HSE manager.

11.2.2.4 Confined Space Entry Rescue Personnel (Emergency Response Team)


1. Attend incident site urgently.
2. Follow instructions of team leader and Site Manager.
3. Coordinate with site manager the responses in emergency cases in the confined spaces.
4. Communicate to the PMT HSE whenever any emergency occurs.
5. Audit the effectiveness and preparedness of the Emergency Response Team
6. Seek advice from Safety Supervisor.
7. Organize and control overall performance of the Emergency Response Exercises.
8. Ask for back-up if required.
9. Be responsible for all required documents and reporting system.
10. Make recommendations, if necessary, on any point.

11.2.2.5 HSE Supervisor


1. Know and apply the ERP and ensure it is brought to the notice.
2. Ensure all staff work in a safe manner and use all protective devices and procedures.
3. Advise all employees of any potential or actual dangers and how to isolate, prevent
or remove them.
4. Ensure that new employees understand, and comply with the ERP.
