I MANMOHAN SINGH S/o INDER SINGH R/o B-2, HANUMAN MANDIR ROAD,
DCM COLONY, BADARPUR MAJRA, BURARI, NORTH, NEW DELHI, DELHI-
110084, Owner of Vehicle Regn. No. DL1LX1642 do hereby solemnly affirm and
declares under:

1. That the vehicle No.DL1LX1642 is not black listed by Delhi Police at any
given time.
2. That the vehicle No.DL1LX1642 is not involved in any fatal road accident
in the last one year.
3. That vehicle No. DL1LX1642 is not prosecuted for misuse of ‘No Entry
Permission’ by Delhi Police.
4. That no Notice/Challan/Suspension order is pending in respect of the
above mentioned vehicle.
5. That I applicant is not involved in drunken driving and no challan is
pending under drunken driving of the above mentioned vehicle.
6. That RUPD & LUPD have been fixed on the vehicle as per provision of
rule 124 CMVR, 1989.
7. That the reflective strips of two inch width on both sides & at the rear
side of the vehicle have been fixed.
8. All supporting documents are genuine and nothing has been concealed.
