No. PHFMC/HR/8626/2024, DR. MAVRA ZAIDI (CNIC: 31203-*******-4), is hereby engaged on Locum Tenens Basis for providing
services at Basic Health Unit, Jahan Pur, Jalalpur Pirwala, Multan. You are directed to report to the office of the Chief Executive
Officer (DHA), Multan within 3 working days. You will be paid for your services provided at the above-referred health facility as per
locum policy approved by the BoD of Punjab Health Facilities Management Company (PHFMC) (as amended from time to time).
It is mandatory for you to get yourself registered on Biometric/Facial Attendance System, Duty Roster of Health Facility (as Locum
Tenens HR) and you will also enter the prescriptions on Electronic Medical Record and any other digital systems of Primary &
Secondary Health Department as proof of services actually delivered in terms of your engagement with Punjab Health Facilities
Management Company (PHFMC).
You are further advised to share your banking details for subsequent payment of your services provided as Locum Tenens HR.
Please note that Locum Tenens arrangement does not guarantee any minimum workload. However, as per the requirement, Punjab
Health Facilities Management Company (PHFMC) will be using your services. In case you are assigned a particular duty, unauthorized
omission to perform your duties on three consecutive occasions may delist you from the panel of Locum Tenens pool.
