AFFIRMATION

I, YYY, of legal age, single, Filipino, and currently


residing at Unit 1 ABC Condominium, Timog Avenue, Quezon
City, hereby respectfully state and affirm:

1. That I am one of the counsels for the accused Rizalito


Cabrera;

2. That accused Rizalito Cabrera is presently bedridden


after suffering from a stroke and is unable to personally
appear before the Honorable Court;

3. That I have personally conferred with accused Cabrera


and have explained to him, in a language and manner
he fully understands, the nature, legal implications, and
effects of a provisional dismissal of the instant criminal
case;

4. That after being so informed and fully apprised,


accused Cabrera expressly gave his voluntary,
informed, and unqualified consent to the provisional
dismissal of the case; and

5. That this Affirmation is executed in compliance with the


directive of this Honorable Court, as stated in its Order
dated June 3, 2025, and to attest to the truthfulness of
the foregoing representations.
