AFFIDAVIT OF LOSS

I, _________, of legal age, Filipino, single and a resident of Prk.12,


Talomo Proper, Davao City, Philippines, after being duly sworn in accordance
with law, hereby depose and say THAT :

1. I am herein affiant;

2. Sometime on 26 November 2022, my nephew, _________, issued a


solicitation letter for his year book for this coming 23 January 2023
graduation;

3. I placed the above-mentioned solicitation letter, inside an envelope which


was kept safe inside the cabinet;

4. However, recently, I discovered that the above mentioned solicitation letter


was no longer inside the envelope in the cabinet;

5. Despite diligent efforts, the same could not be found, hence, declared lost
for all legal intents and purposes;

6. I am executing this affidavit in order to attest to the truth of the foregoing


facts and to inform the Davao Merchant Marine Academy (DMMA) and for
whatever purpose it may serve.
