Good day.

We would like to inform you of an incident involving your daughter, Ms. Angelica Ani, that
occurred on August 1, 2025, at approximately 3:00 PM.

Despite reportedly not feeling well, Angelica insisted on participating in practice activities on the
said date. Unfortunately, due to her physical condition, she almost fainted during the session
and was immediately given first aid. She was then brought to the hospital for further medical
attention, accompanied by a parent/guardian.

Please note that at the time of the incident, her designated coordinator had an ongoing class in
another classroom and was not immediately present during the occurrence.

We sincerely hope for Angelica’s full and speedy recovery. May we also take this opportunity to
kindly remind students and parents of the importance of prioritizing one’s health and well-being.
If a student is feeling unwell, we highly encourage them to rest and seek medical attention before
resuming any physical or extracurricular activities.

INCIDENT REPORT

Name of Student Involved: Angelica Ani
Grade/Section: Grade 9 - Courage
Date of Incident: August 1, 2025
Time of Incident: Approximately 3:00 PM
Location: SDI Grounds

Description of Incident:

On August 1, 2025, at around 3:00 PM, student Angelica Ani almost fainted during a scheduled
practice session. It was noted that she was not feeling well prior to the activity but insisted on
participating despite her condition.

Shortly into the session, Ms. Ani most lost consciousness. Immediate assistance was provided,
and first aid was administered. She was promptly brought to the hospital for medical evaluation,
accompanied by her parent/guardian.

At the time of the incident, her designated coordinator was attending to a class in another
classroom and was not present when the incident occurred.

Action Taken:

 First aid was administered on-site.


 Parent/guardian was contacted immediately.
 Student was escorted to the hospital by her parents/guardian.
 Incident was reported to the school administration.

Remarks/Recommendations:

It is recommended that students be reminded to prioritize their health and inform teachers or
coordinators if they are feeling unwell. The school will continue to reinforce its health and safety
protocols during physical activities.

INCIDENT REPORT

Name of Students Involved: Marx Tanay, Cyrus Retreta
Grade/Section: Grade 8 – Unity
Date of Incident: July 10, 2025
Time of Incident: Approximately 10:45 AM
Location: Grade 8 Unity

Description of Incident:

On July 10, 2025, during the scheduled Filipino class between 10:45 and 11:45 AM, a physical
altercation occurred between two Grade 8 – Unity students: Marx Tanay and Cyrus Retreta.
Based on witness accounts, the teacher arrived approximately three (3) minutes late for class.
During this short window, a disagreement between the two students escalated quickly into a
physical confrontation.
According to statements from classmates, Cyrus Retreta was the initial aggressor, which
triggered the incident. Immediate intervention was made by the teacher upon entering the
classroom, and both students were separated and calmed down.
A discussion between the involved students, their adviser, and their Filipino Teacher was
conducted on the same day. Both Marx and Cyrus mutually agreed to reconcile and resolve the
conflict peacefully to prevent any further issues.

Action Taken:
• Students were separated and spoken to individually by the Filipino teacher and adviser.
• The incident was referred to the Principal office for proper documentation and intervention.
• A mediation session was conducted with both students present.

Remarks:
Students reminded of appropriate behavior and conflict resolution techniques, especially during
unsupervised moments.
