 Safety is the state of being "safe", the condition


of being protected from harm.
. An example of safety is when you wear a seat
belt. An example of safety is a safety belt.
❑ Patient safety is the avoidance of unexpected
harm to people during the provision of health
care. Patients should be treated in
a safe environment and protected from harm.
 Blood borne Pathogens
 Hazardous Chemicals
 Slips and Falls
 Latex Allergies
 stress
 It is an entry of a client (patient) into the
hospital ward for the therapeutic (treatment) or
diagnostic (investigation) purposes.

PURPOSES
 To give immediate care, safety and comfort.

 To observe sign (observe) and symptoms


(patient told you) and general condition.
 To help in finding any health problem.

 To diagnose any health problem.


 Receive patient with courtesy.
 Check admission slip of the patient.
 If patient is walking lay him/her on chair/stool.
 Provide him bed to lie on the bed for feeling comfort.
 Orientate patient about kitchen, washroom Dr/staff room etc.
 Instruct patient and their attendant to keep valuable on safe place.
 Check doctor's orders and then carry out the doctor's orders carefully.
 Check patient vital signs and also body weight (wt) and height and record it.
 Prepare a chart fill all information E.g.: OPD no, ward no, pt name, father
/husband name, age, sex, ward register no, bed no, and DO (date of) admission,
DO (date of) discharge etc. inform client about visiting hours and rules and
regulation of hospital during visiting hours.
 Collect sample of blood, sputum or urine of client for purpose of investigation if
required.
 If Operation is needed then take the written consent from patient if conscious
otherwise take from the attendant.
 If any medicine is required give a slip to attendant to purchase it from medical
store.

 DISCHARGE PROCEDURE:
 Inform patient about discharge in advance.
 Never discharge patient unless the discharge card (DC)
is not made by doctor.
 Investigations and other record should be provided to
pt with DC keep copy in hospital file.
 Brief explain about route and right of medication, diet
and next date of visit to hospital for checkup in written.
 Ask patient to bring DC with them on visit to hospital
for checkup.
 If patient is unable to walk then give him wheelchair at
the time of discharge to reach toward ambulance or
transport.
 Goodbye to the patient with wishing of quick recovery.
 Return back to bed remove linen and clean bed
