AFFIDAVIT
1.DENNIS KINYA NGARUIYA, of post office box 010-545, THIKA within
republic of Kenya do hereby make Oaths and state as follows: -
1. THAT I am a male adult of sound mind and holder of card
NO.39936170 hence competent to swear this affidavit.
2. THAT I would like to clarify that the name DENNIS KINYA
NGARUIYA as written erroneously on my logbook refers to me.
3. THAT also in my Pin Certificate it was again written erroneously as
DENNIS KINYA NGARUIYA.
4. THAT both names refer to me, I am one and the same person.
5. THAT I swear this affidavit conscientiously believing everything
contained herein is true and in accordance with Oaths and statutory
declaration Act CAP 15 of the laws of Kenya.
6. 6.Sworn at Thika by the said DENNIS KINYA NGARUIYA
at THIKA on this day of 2025
