Abstract:

Introduction: Cervical cancer is the fourth most common cancer in women globally with an estimated
604 000 new cases and 342 000 deaths in 2020.The highest rates of cervical cancer incidence and
mortality are in low- and middle-income countries 1. Cervical cancer ranks as the 13th most frequent
cancer among women in Egypt and the 9th most frequent cancer among women between 15 and 44 years
of age2.
Objective: the objective was to determine the efficacy of neoadjuvant chemotherapy in cervical cancer.
Methods: a prospective cohort study was conducted on 65 patients, who were diagnosed as cervical
cancer patients with stage IB1 to stage IIB from the period of 2018 till 2022. They received neoadjuvant
chemotherapy in the form of paclitaxel and cisplatin over 9 weeks followed by reassessment of its success
in the form of downstaging of cervical lesions. The down staged cases were candidate for radical
hysterectomy and pelvic lymphadenectomy.
Results: within the 65 cases, 62 cases had successful down-staged and operation was done. The
respectability of the tumor with safety margin was evaluated, also the operative complication was
reported as the major complication were bladder and ureteric injury with present of 6.2% each.
Complication of chemotherapy was reported, the most common as pancytopenia (12.8%) that was
reversible after stopping chemotherapy. Follow up the patients for 2 years with relapse occurred in 10.7%
of cases.
Conclusion: neoadjuvant chemotherapy was successful in downstaging locally advanced cervical cancer
and reduce the need of adjuvant radiotherapy after surgery.
