Complete the assessment to determine your longevity. The assessment also includes suggestions for
improving your health. Based on your assessment results and suggestions provided, answer the following
questions:
1. (5 POINTS) - How long are you expected to live based on your current health habits?
2. (5 POINTS) - What are some of your good health habits?
3. (5 POINTS) - List some future intentions for improving your health habits and extending your health life
span.
Part II
Go to http://www.choosemyplate.gov/ to find information on your nutritional needs. Complete the MyPlate
Quiz (scroll down towards the bottom of the page) and then answer the following questions:
1. (5 POINTS) – Based on your results, what type of MyPlate user are you (Beginner, Rookie, Apprentice,
Pro, All Star, or Hall of Famer)?
2. (5 POINTS) – Based on your results, what food group(s) do you need to eat more often, if any?
3. (5 POINTS) – What was your 6-digit results code (scroll down to the “Free resources and tools for you”
section, and view your results code under Start Simple with MyPlate App)? Feel free to download the
MyPlate App and use this 6-digit code to set your food group goals, if you like.
4. (5 POINTS) – Go to https://www.myplate.gov/myplate-plan and click on Start for the Get Your MyPlate
Plan. Fill out the required information to obtain your recommended caloric intake. Then, click on your
results to receive the recommended serving sizes of each food group. What are your recommended
serving sizes for each of the five food groups?

Part III
Physical fitness is also an important element to healthy living. Based on Chapter 50 in your textbook, make a
weekly exercise schedule and commit to doing it the week after the course ends and beyond.
1. (5 POINTS) - List your weekly exercise schedule. Be sure to include what you will be doing and for how
long or the number of reps you will be doing.
