Event Participation Waiver - Table Tennis Ace Championship
I, the undersigned, hereby acknowledge and agree to the terms and conditions outlined in this
Event Participation Waiver (“Waiver”) in consideration for my participation in the Parlor Non
Profit Table Tennis Tournament (the “Event”) organized by Parlor Non Profit (the “Organizer”).
1. Assumption of Risk: I understand and acknowledge that participating in the event
involves certain inherent risks, including but not limited to physical injury, property
damage, and other unforeseen dangers. I voluntarily and willingly assume all risks
associated with my participation in the event.
2. Release of Liability: In consideration for being allowed to participate in the event, I
hereby release, waive, discharge and covenant not to sue the Organizer, its officers,
agents, employees, volunteers or any other representatives from any and all claims.
3. Compliance with Rules: I agree to comply with all rules and instructions provided by
the Organizers or their representatives during the course of the event.
4. Health Declaration: I am not aware of any injury, illness, or other health-related issues
that would restrict or limit my ability to play competitive sports.
5. Consent for Use of Photographs: I grant permission to the Organizer to capture and
use photographs, video recordings, or any other media of my participation in the Event
for promotional, marketing, and event-related purposes. I understand that these
materials may be used in print, digital, and social media formats without compensation or
further approval. If I do not wish to be photographed, I will notify the Organizer in writing
prior to the event.
6. Acknowledgment of Understanding: I have read this release waiver, fully understand
its terms, and acknowledge that I am giving up substantial rights by signing it. I further
acknowledge that I am signing this document freely and voluntarily, without any
inducement, coercion, or undue influence.
I certify that I am the age of majority (18 or 19, dependent on the province or territory) and have
read and understood the terms of this waiver. If I am under the age of majority, my parent or
legal guardian has read and understood this Waiver and has executed it on my behalf.
