WAIVER OF APPEARANCE
The undersigned accused XXX most respectfully WAIVE HIS
APPEARANCE during the trial of the above-entitled case, under the
following terms and conditions:
1. To appear before this Honorable Court whenever required,
and that in case of failure on his part to do so without justification
despite due notice, such failure shall be deemed an express waiver of
his right to be present on the date specified in the notice, and trial
may then proceed in absentia;
2. To surrender himself for the execution of the final
judgment;
3. To desist and refrain from leaving the country without
prior permission from this Honorable Court;
4. To notify this Honorable Court of any change in their
present residence at XXX.
5. To strictly abide by the foregoing undertaking during the
entire proceedings of this case until its final termination; and
6. As a standard condition imposed by this Court, every time
his name is mentioned in the course of the trial, he agrees that said
name pertains to him and that he will not dispute his identity with
such name.
WHEREFORE, it is respectfully prayed that this WAIVER OF
APPEARANCE be entered and noted in the records of this case.
