1. This investigation ga on was conducted pursuant to the institution on of chief of police of Dipolog
City Police Station, Jonathan Javier, about the Stealing incident at LKS Department Store on September
5, 2025

4. To investigate the facts and circumstances our team has been working delightfully and
analyzed the incident scene. We interview witnesses and obtain CCTV footage inside the establishment
related to the incident, we collect marks and even photograph the evidence.

7. On September 5, 2025. Prior to the investigation the victim owner of the
department store is complaining about stealing incident inside his store. This happened on Sept.
05,2025 the victim caught one of his worker stealing goods and money caught in CCTV footage.
IV. Discussion and Findings
8. An assessment of the scene suggests a filing of complain against the suspect
And get his identity.
V. Conclusion
a) 9. After careful perusal of circumstances surrounding this particular incident, the suspect
identified Adamiel Bryant P. Santos, 22 years old, LKS clerk, resident of Brgy. Barra,
Dipolog City that caused the crime.
b) 10. Therefore in view of the foregoing this office hereby Penalties range from minor
punishment for low-value items to imprisonment for longer periods and increased fines for
more valuable stolen goods. Be filed against Adamiel Bryant P. Santos, 22 years old, LKS
clerk, resident of Brgy. Barra, Dipolog City.

1. Request conducts polygraph examination on the following person who
were suspected responsible in stealing incident that transpired inside the premises of
LKS Department Store Dipolog Branch, Central Brgy, Gen.Luna St, Dipolog City on or
about 5;40 p.m. of September 5, 2025, and on or 4:50 p.m. to wit :
c) NATURE OF CASE: Theft
d) VICTIM: Jonathan Javier,36 Years Old, Owner of LKS Department Store, Resident
of Brgy. Estaka Dipolog City;
e) SUSPECT: Adamiel Bryant P. Santos, 22 years old, LKS clerk, resident of Brgy.
Barra, Dipolog City.
2. Further request that this Office be furnished a copy of examination result
for our reference.
3. Hoping for your priority and favorable action on the matter.

AFFIDAVIT OF WITNESS
I, Zee Brendia 22 age, single , resident of
Dansullan Polanco, after having sworn in accordance with law, do
hereby depose and state THAT:
01. I was present and personally saw that accused, that the suspect , committed the
crime of theft against the victim Jonathan Javier owner of the department store.
02. That on sept 5, 2025, I saw Adamiel stealing money on the cash register and even on
the stock room stealing goods
03. That when we were in the stock room adamiel always steal goods and put it on the
eco bag and hide it so no one can see it there. I immediately told our boss what I saw
04. That when I saw it I report to the owner and he immediately go to the CCTV footage
and review all the CCTV cameras on the store and find out that adamiel is actually stealing
goods and money on his store.
I executed this affidavit to attest the truthfulness of the foregoing facts and to support
the filing of Criminal Cases against Adamiel Bryant P. Santos for violations of Theft .
