CERTIFICATE
Certified that: -
(i) The afore said examination was conducted on 04 May 2025 (Sunday) at
the above-mentioned Exam Centre smoothly and fairly in accordance with
the Guidelines / Instructions issued by NTA.
(ii) The Sealed Trunk(s) for the aforesaid Centre, containing sealed Packets of
Question Paper Booklets with OMR Answer Sheet inside, was collected
from the City Coordinator alongwith the Centre Superintendent from SBI on
04 May 2025 (Morning) in closed vehicle and the same was kept in the Control
Room of the Exam Centre under CCTV Surveillance/strict supervision.
(iii) The sealed Trunk(s) was opened in the Control Room at 01:15 PM on 04 May
2025 in the presence of the undersigned, Centre Superintendent, 02
Invigilators and 02 Candidates.
(iv) The sealed Packets of Question Papers (with OMR Answer Sheets inside)
were distributed to the Invigilators for respective exam rooms in our presence
at the Control Room as per instructions.
(v) The Question Paper Booklet (with OMR Answer Sheet inside) was distributed
by the Invigilators concerned to the candidates present in the exam rooms at
appointed time (01:40 PM).
(vi) The exam started and concluded at the appointed time [at 02:00 PM & 05:00
PM respectively for Non-PwD/PwBD candidates and from 02:00 PM to 06:00
PM for PwD/PwBD Candidates (wherever applicable)].
(vii) The no. of candidates who have appeared in the abovesaid exam centre is
_____231_______ out of _____240__________scheduled. The no. of
candidates absent is __09________
(viii) The unused test booklets (09 number) of absentee candidates, collected from
the exam rooms after the commencement of the exam, were kept in safe
custody in the control room till the exam was over.
(ix) The original OMR answer sheets brought by Invigilators in pink envelopes in
sealed condition from the exam rooms by the room invigilators alongwith the
packet of office copies of OMR answer sheets as well as the attendance sheets
and admit cards, were kept inside the trunk (s) as per the packing instructions,
before the same was handed over in sealed condition to the City Coordinator at
the Collection Centre on 04 May 2025 evening.

(x) There was no incident of any person indulging in impersonations, cheating or
use of unfair means by any candidate at the aforesaid exam centre.

indulging in impersonation or any candidate adopting any unfair means, cheating,
etc. full report about the matter including the inquiry made and the action taken
thereon be given by the Observer(s) in a separate sheet.]
