NO 498, s. 2025 and other relevant issuances.

a. Checklist of Requirements and Omnibus certification of authenticity and veracity of all
documents submitted,

b. Letter of Intent addressed to Schools Division Superintendent
c. Duly accomplished Personal Data Sheet (PDS) with recent passport-sized picture with work
experience sheet.
d. Photocopy of valid and updated PRC License/ID;
e. Transcript of records
f. Certificate of Competency Level
g. Photocopy of duly signed Service Record;
h. Photocopy of latest appointment
i. Photocopy of certificates/s of completion of National Educators Academy of the Philippines
(NEAP)-
accredited public and private institutions or photocopy of certificates of relevant specialized
trainings and
professional development programs.
j. Certified copies of Performance Rating for the last three (3) rating periods
k. other documents as may be required by the HRMPSB including but not limited to portfolio for
the assessment

Teacher VI in Jasminez Elementary School ,Division of Samar.
My primary purpose in applying is to further advance my career in the Department of Ed
ucation and to share my expertise, leadership, and dedication to teaching and learning.

uality education by demonstrating effective teaching practices, mentoring colleagues, an
