Laverta Boykins was evaluated in the Emergency Department on 09/12/2025 following a motor vehicle
collision. Examination and clinical assessment were completed, and the patient was discharged in stable
condition with home care instructions.
Plan & Instructions:
•The patient may use over-the-counter pain relievers as directed and activity as tolerated.

• Follow up with a primary care provider or urgent care in 1–5 days (or sooner) if any new or persistent
pains develop, including headache, neck/back pain, abdominal pain, chest pain, or increased soreness.

• Return to the ER immediately for worsening symptoms, severe headache, repeated vomiting, weakness
or numbness, confusion, trouble breathing, chest pain, high fever, or any other concerning symptoms.
